Instructions for submitting an overtime claim form

As one form per week. If you run out of room, please use an additional form. 

A key explaining the overtime elements is shown below with a brief description of what each claim means.

If inputting overtime that includes an unpaid break, this should be input in separate rows. 

Guidance for Managers processing electronic submissions

The Manager is responsible for checking each employee claim is correct and all relevant fields are input correctly for Audit purposes before transferring to the Manager Input form. Only one Manager input form should be submitted per service. We recommend that forms are submitted to Shared Services/Payroll on a weekly basis to ensure prompt processing.

  • Copy the following from row 6 into the Manager Input Form Each employee should submit their own Employee Claim form to their manager and is responsible for ensuring that each relevant section is completed, both electronic and paper forms are available.

  • Copy the detail under 'Grade', 'Overtime description', 'iTrent Ref' and 'Date Worked'.

    You must ‘Copy’ the information, then right click and use ‘paste’ "values" to paste in the relevant details.
